1. Soak bread or cracker crumbs in milk.
2. Mix meat, seasonings, and soaked crumbs thoroughly.
3. In a separate bowl, mix sour cream and mushroom soup.
4. Make meat mixture into loaf and pour the sour cream-soup mixture over top,.then add ketchup
5. Cover; cook on Low heat setting 3 to 4 hours or until meat thermometer inserted in center of loaf reads 160°F..
